I believe that in many cases, failure boils down to the following:

1. Infrastructure - Lack of an efficient, streamlined, and current finance backbone, that provides solid data in real time.

2. Direction - Lack of a well thought out direction and strategy, broken down into action items, and measured constantly against KPIs and milestones.

3. Discipline - Lack of discipline to follow the above steps.
